Hawaiʻi State Identification Cards

**Hawaii State Identification Cards are available at the Hilo, Kona and Waimea Offices only.

Effective January 02, 2013, anyone applying for any original or renewal of their Hawaii State Identification Card must present original or certified documentary proof of:

  • Legal name
  • Date of birth
  • Social security number
  • Legal presence and
  • Proof of principal residence address

The application of a minor applicant between the age of 14 through 17 must include a signature from a parent or by another individual in loco parentis of the minor who can provide proof of guardianship. A certified copy of the minor’s birth certificate is required to verify name of parent.
