A completed Appointment of Persons in Active Charge of the Premises form is required to sign up employees for the liquor examination.   After you submit the completed form, you will be sent confirmation of the session date and time via email or phone.

NOTIFY YOUR EMPLOYEES THAT:

• Sessions are at least 2 hours (includes a workshop and exam).
• In-person registration starts 30 minutes prior to session time.
• A $2.00 fee will be collected for the liquor card, cash only, bring exact change.
• Valid government-issued photo ID must be presented at the time of registration.
• Employees age 18 -20 may attend if space is available but cannot be a Person In Active Charge.
• For parking in the Hilo Lagoon parking lot, have your license plate number at registration.
